Services for Dwayne Eldon Sage, 43, of Desoto and formerly of Des Moines, who died Sunday at Mercy Hospital Medical Center of a heart attack, will be at 1:30 p.m. Wednesday at Hamilton''s Funeral Home, with burial in Highland Memory Gardens Cemetery.

Born in Bedford, Mr. Sage lived in Des Moines 19 years before moving to Desoto six years ago.  He was a foreman for the forestry division of the Des Moines Parks and Recreation Department and a member of Berean Assembly of God Church and Laborers Union Local 353.

Survivors include his wife, Betty; a daughter, April Sage, at home; a son, Clifford, at home; his father, Otto Sage of Polk City; a sister, Alice Duff of Des Moines, and three brothers, Kenneth of Ankeny, Keith of Woodridge, IL, and Dwight of Desoto.

The family suggests memorial contributions to the Shriners Crippled Children''s Hospital.
